20-minute Physical Therapy evaluation: clinical decision of low complexity, no personal factors, comorbidities. activity limitations or participation restrictions can impact the plan of care
30-minute Physical Therapy evaluation: clinical decision of moderate complexity, a history of present problem with 1-2 personal factors and/or comorbidities that impact the plan of care, examination of body systems using standardized tests and measures in addressing a total of 3 or more elements from body functions, activity limitations, and/or participation restrictions;
45-minute Physical Therapy evaluation: clinical decision of high complexity, a history of present problem with 3+ personal factors or comorbidities that impact the plan of care; examination of body systems using standardized tests and measures addressing a total of 4+ elements from body functions, activity limitations, and/or participation restrictions;
15-minute Therapeutic Activities, one-on-one contact to improve functional performance, including bending, lifting, carrying, reaching, catching and overhead activities to improve functional performance in a progressive manner.
